Most systems in this space capture support well. What is usually missing is what happens to a note afterwards — who checked it, who can read it, and what happens when it turns out to be wrong.

The pain

What this feature fixes.

Nobody checked it before it became the record.

A note written at 11pm by a tired relief worker is, from that moment, the evidence. Having a senior see it and question it before it hardens is the half that survives a review.

One notes field, two incompatible duties.

The person has a right to their record and a right not to be handed a risk assessment about themselves. A single free-text field cannot hold both, so the honest record ends up somewhere they can never see.

A correction destroys the original.

Something recorded in error gets edited. The earlier version is gone, and with it any ability to show what was known at the time and what changed.
